BELLWOOD, Ill. (AP) — Federal workplace regulators are proposing more than $77,000 in fines against a suburban Chicago industrial-cleaning company after a worker died cleaning a storage tank last November.
The U.S. Department of Labor’s Occupational Safety and Health Administration said Monday it has cited Bellwood-based Phoenix Industrial Cleaning for 28 allegedly serious safety violations as part of the agency’s probe.

OSHA says the worker died after falling from a ladder inside the storage tank at a Wheeling chemical-making site after being overcome by methylene chloride vapors. That chemical commonly is used as a degreaser and paint remover in industrial and home-improvement products.
Phoenix Industrial Cleaning didn’t immediately return a message Monday seeking comment, including whether it would request an informal conference with OSHA’s area director, challenge the citations and penalties, or comply.
